To begin the credentialing process, please contact the Operation Smile Ireland Credentialling Department – or 01 667 6659.

Please be sure to mention your current occupation or area of specialty so we can send you the correct form!

You will be sent the relevant application form for your speciality. Please complete and return the application form, along with all supporting documentation.
